Medical Records for Legal or Court Matters
Dr. Gao's office does not provide professional or litigation-related services on a contingency basis. Payment is not dependent upon the outcome, settlement, award, or resolution of a legal claim or court proceeding.
Medical Records
Copies of existing patient medical records are provided upon receipt of an appropriate written request or authorization and are subject to applicable Connecticut and federal laws governing medical-record access, processing times, and permitted copying fees.
Legal Reports & Professional Services
Requests related to litigation that require services beyond providing copies of the existing medical record—including preparation of narrative reports, review of records or other materials, attorney consultations, affidavits, depositions, testimony, or other professional services—are subject to a separate professional fee. The minimum fee for these services is equivalent to the current fee for one treatment visit. Additional fees may apply when substantial preparation or professional time is required.
